Greensboro Woman Charged After Larry Burton Goes Missing From Unlicensed Care Home

A Greensboro woman, Hazel Mae Forman, has officially been charged for operating an unlicensed adult care facility after 74-year-old Larry Darnell Burton vanished from her property in early July.

Forman, 50, turned herself in Friday and later posted a $10,000 bond, according to court documents. She was charged with Running an unlicensed care home.

Where is Mr. Larry Burton?

Larry Burton was transferred from Moses Cone Hospital to L&L Family Care Home — run by Forman — on June 24, 2025. He was supposed to be getting post-hospital care. But just over a week later, he was gone.

As we previously reported, Burton’s daughter, Latonya Swift, told us that Hazel Forman called her on the morning of July 3, claiming Burton had been seen asleep at 5:00 a.m., but by breakfast time — 7:00 a.m. — “he was gone.”

STATE SAYS CARE HOME WAS ILLEGAL

The North Carolina Department of Health and Human Services (NCDHHS) investigated and confirmed that L&L Family Care Home was operating without a license — a serious violation under state law.

After officials told Forman to shut it down, residents were still found living in the facility weeks later, according to WXII.

The Guilford County DA’s Office is reportedly reviewing further criminal charges, and the case remains under active investigation.

CHARGES, BUT STILL NO LARRY

So far, Forman is only facing the unlicensed care facility charge, and no neglect or endangerment charges have been filed — yet. Authorities stress that the search for Larry Burton is still ongoing.
